CNA JOB DESCRIPTION

A CNA at St. Clare Living Community is responsible for providing personal cares in compliance with each resident’s individualized care plan, as well as observing and reporting changes in a client’s condition and/or needs. A CNA will work under the supervision and direction of a Registered Nurse or License Practical Nurse.

EXPERIENCE AND/OR EDUCATION REQUIREMENTS

Applicants must be in good standing and currently active on the MN Nursing Assistant Registry.

SKILLS/KNOWLEDGE REQUIREMENTS

Applicants must have a genuine interest in geriatrics and a desire and willingness to further his/her experience and education. Patience and tact in dealing with residents and their families and visitors is a must. Initiative and judgment in observing changes in client’s conditions, ability to keep residents information confidential as per HIPAA, and the ability to perform a variety of simple repetitive tasks are all requirements of this position. Applicants must have the ability to work independently and take initiative as well as being flexible and able to adapt to changing situations. Applicants are required to complete 12 hours of in-service annually and must attend all mandatory facility meetings.

ESSENTIAL FUNCTIONS OF THE JOB

  1. Performs initial and ongoing data collection on all patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.

  2. Implements plan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural, ethnic and religious/spiritual needs.

  3. Provides direct patient/resident care and reports condition and changes to RN or LPN.

  4. Demonstrates time management and prioritization of tasks.

  5. Documents results of patient/resident care within established policies, procedures and scope of practice.
