LPN - RN JOB DESCRIPTION

St. Clare Living Community is currently seeking an LPN or RN who provides care to a group of patients and residents under general supervision of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interact with patients and residents, families, responsible party, and all health team members.

EXPERIENCE AND/OR EDUCATION REQUIREMENTS

  • Current state licensure as a LPN or RN, which must be in good standing

  • Must have good communication skills

  • Must be able to read, write and communicate in English

  • Clinical experience preferred

SKILLS/KNOWLEDGE REQUIREMENTS

Required to analyze data, and remember and recall multiple policies and procedures. Must have organizational skills, ability to revise and implement the plan of care and observe record and report on patient/resident status. Must be able to demonstrate multiple priorities and the ability to direct and evaluate the activities of others.

ESSENTIAL FUNCTIONS OF THE JOB (RN)

  • Assessment: Performs assessments on patients/residents and reassessments as per policy.

  • Planning/Implementation/Evaluation: Develops and implements appropriate plan of care based on assessment and diagnosis tailored to meet each individual’s needs (physical, cultural and psychosocial), evaluates plan of care and makes necessary changes, works to meet individual patient/resident needs.

  • Medication Administration: Demonstrates knowledge of medications and their correct administration based on age of the patient/resident and their clinical condition. Evaluates patient/resident responses to medications.

  • Patient/Resident/Family Education: Plans for and provides patient/resident, family and caregiver education, identifies learning needs, clearly communicates information and documents patient/resident response to education.

  • Documentation: Documents according to current standards and policies.

  • Discharge Planning: Plans for patient/resident discharge, anticipates and plans for long-term and/or short-term patient/resident needs, collaborates with the discharge planner when appropriate and assesses the patient/resident prior to discharge.

  • Delegation/Collaboration: Delegates and collaborates in providing patient/resident care according to licensure and skill levels, follows up to ensure success.

ESSENTIAL FUNCTIONS OF THE JOB (LPN)

  • Data Collection: Performs initial data collection on all patients/residents and follows up on data collection as per policy requirements.

  • Implementation: Participates in implementing age and developmentally appropriate plan of care as developed by the Registered Nurse within established policies and procedures and continually works to meet individual patient/resident needs.

  • Medication Administration: Demonstrates knowledge of medications and their correct administration based on age of the patient/resident and their clinical condition. Observes, documents and reports patient/resident responses to medications.

  • Patient/Resident/Family Education: Follows patient/resident, family and caregiver education plan of care as developed by the Registered Nurse, clearly communicates information and documents patient/resident response to education.

  • Documentation: Documents according to current standards and policies.

  • Discharge Planning: Plans for patient/resident discharge, collaborates with the Registered Nurse and discharge planners for long and short term patient/resident needs.

  • Coordination/Direction: Coordinates and directs patient/resident care appropriately. Performs charge nurse duties as assigned.
